The invitation to the screening arrives in the Diya app on the 30th day after the person's birthday (if the person is 40 years old or older).
After the person accepts the invitation, they need to order a Diya Card in the app or use their existing one.
Within 7 days, the card will be credited with 2,000 hryvnia. These funds can only be used to undergo the 40+ Health Screening at a specific list of institutions.
If a person does not have the Diya app, they can still take advantage of the program. To do so, they must submit an application through the Administrative Services Center. The person orders a plastic card with a special account at designated banks (currently PrivatBank), contacts the ASC, and submits an application to participate in the program. After about 7 days, the person receives an email notification that the funds have been credited and then continues the process in the same way as other program participants — they choose an institution and undergo an examination.
After receiving the funds, the person makes an appointment at a medical institution participating in the program. The list of medical institutions is published on a special landing page. http://screening.moz.gov.ua.

After selecting a convenient institution on the landing page, a person can immediately proceed to registration through the corresponding functionality of the medical information system (we recommend choosing this option).
You can also make an appointment using the web form on the website of the selected medical facility, by phone through the reception desk, or in person.
Medical facilities must designate separate time slots specifically for health screening. The appointment system must mark these slots as available only for health screening.
After making an appointment, the person should receive a confirmation (SMS, push notification, or email) clearly stating the date, time, and address of the health screening. A certain amount of time before the visit (24–48 hours) should a reminder be sent with the option to cancel or reschedule the appointment online, as well as recommendations on how to prepare for the health screening.
At the appointed time, the person should visit the medical facility for screening. No special preparation is required for screening. However, it is recommended to refrain from intense walking, smoking, drinking tea, coffee, and other caffeinated beverages for at least 30 minutes before the screening.
The visit lasts 60–90 minutes, depending on the scope of medical services. The duration of a basic visit is up to 60 minutes (questionnaires, examination, biomaterial collection, basic consultation).
During the visit, the person pays for the service with a Dія.Card.
No later than 72 hours after the visit, the patient must be sent the results of laboratory tests (lipid profile, HbA1c, and other indicators). Various channels can be used for this: the patient's account in the MIS, email, messengers.
The indicators should be presented in a "traffic light" format:
🟢 normal, with a brief explanation that everything is fine;
🟡 borderline values – a brief explanation that requires attention;
🔴 pathological condition – a brief explanation and instructions for further steps (contact your family doctor, undergo additional examination, etc.).
Important! After passing the screening and paying for the service with the Dія.Card, the funds will be automatically debited from your card. If you do not use the service, the funds will automatically be returned to the state budget, and you will only be able to undergo screening again next year.
