Assistance is provided to individuals, including sole proprietors (FOP), who:
▫ have activity codes (KVED) 97.00, 88.91, 85.59, or 87.90;
▫ have expressed the desire to become professional foster carers;
▫ can provide a child with proper living, learning, and development conditions;
▫ have an adult family member (spouse, son or daughter, another family member) or another adult willing to act as an assistant foster carer, undergo training together with the foster carer candidate, and help care for the child placed under foster care.
📄 Required documents:
🔸 application form;
🔸 identity document;
🔸 taxpayer registration card (RNOKPP);
🔸 order from the Child Services Office on the placement of the child;
🔸 contract on foster care conditions;
🔸 foster care agreement;
🔸 act on the transfer of the child;
🔸 child’s birth certificate or its electronic copy;
🔸 extract from the medical-social expert commission report, decision of the functional assessment team, or medical conclusion from a healthcare facility’s advisory commission regarding a child with a disability (issued in accordance with MoH regulations);
🔸 extracts from the Unified State Register of Legal Entities and the Register of Single Tax Payers (for sole proprietors).
✳ Amount of assistance:
🖇 2.5 subsistence minimums per month for children of the relevant age;
🖇 3.5 subsistence minimums per month for children with disabilities.
✅ Payments are made monthly from the moment the child is placed in a foster family until the day the child leaves.
📬 How to apply:
🔹 at a service center of the Pension Fund of Ukraine — regardless of the registered/declared place of residence;
🔹 by postal mail;
🔹 through authorized representatives of local councils (village, settlement, city, or district-in-city councils, if applicable);
🔹 through an Administrative Services Center (CNAP) — regardless of registered/declared place of residence;
🔹 electronically (if technically possible) — via the Pension Fund of Ukraine’s e-services web portal, mobile app, or the Diia portal.
