Family members of military personnel and combatants are entitled to apply to the Pension Fund of Ukraine, an official of the executive body, or the administrative services center to submit an application and relevant documents for the assignment of benefits for housing and communal services if the beneficiary is unable to apply independently for valid reasons (military service, illness, disability, etc.).
To include information about the beneficiary in the Register, the legal representative or a legally capable adult member of the beneficiary's family who is covered by the benefits shall submit to the territorial body of the Pension Fund of Ukraine an application, copies of documents confirming the family relationship with the beneficiary, or copies of documents confirming the legality of representing the beneficiary's interests, as well as copies of the necessary documents confirming the beneficiary's right to benefits (Resolution of the Cabinet of Ministers of Ukraine No. 373 of 17.04.2019 “Some issues of providing housing subsidies and benefits for the payment of housing and communal services, purchase of solid fuel and liquefied gas in cash form").
The family members of a combatant include:
🔹 spouse (husband), their minor children (under 18 years of age);
🔹 unmarried adult children recognized as persons with disabilities since childhood of groups I and II or persons with disabilities of group I;
🔹 a person who lives with a person with a war-related disability of group I and cares for them, provided that the person with a war-related disability is not married;
🔹 Parents who are unable to work;
🔹 A person who is under the guardianship or care of a citizen who is entitled to benefits and lives with them.
Example of an application by family members of a beneficiary
The mother/father of a combatant or a person with a war-related disability who lives at the same address as the beneficiary shall submit an application for the benefit.
To complete the application, the mother/father of the beneficiary must have the following documents:
🖇 a copy of the certificate of a combatant or a person with a war-related disability;
🖇 the passport of the mother/father of a combatant or a person with a war-related disability;
🖇 the birth certificate of the beneficiary;
🖇 the mother's marriage certificate (if the mother's surname differs from the surname indicated on the birth certificate of the beneficiary son),
and have information about:
🖇 the beneficiary's passport details;
🖇 the beneficiary's identification code;
🖇 the beneficiary's bank account details for the transfer of funds;
🖇 personal account numbers for the payment of housing and communal services;
🖇 the name of the organization providing housing and communal services;
🖇 size of the heated and unheated area of the premises for which the benefit is being applied for in square meters.
