Planning to apply for child benefits? Save this checklist to make sure you have all the necessary documents prepared in advance.
Basic Documents (Required in All Cases)
- A valid identity document.
- A completed application form.
- The child's birth certificate.
- Documents confirming the authority of a guardian, custodian, or adoptive parent (if applicable).
- Bank account details for receiving the benefit.
Additional Documents (Depending on the Type of Benefit)
- Declaration of income and assets.
- Income certificates.
- Court decision on adoption.
- Decision of the guardianship and custodianship authority.
- Medical certificates or conclusions issued by a Medical Advisory Commission (LCC).
- Certificates confirming alimony or scholarship payments.
- Employer-issued documents confirming return to work—for the eYasla childcare benefit.
How to Submit Your Documents
You can apply through:
- Service centers of the Pension Fund of Ukraine.
- Administrative Service Centers (CNAPs) or authorized representatives of local communities.
- Postal services (application and properly certified copies of the required documents).
- The Diia portal.
Certain child benefits can also be applied for through the integrated eMaliatko ("eBaby") service.
If Your Child Was Born Abroad
Documents issued by foreign authorities must:
- be legalized in accordance with Ukrainian law (by apostille or consular legalization, unless otherwise provided by international agreements of Ukraine);
- include an official translation into Ukrainian;
- contain personal data that fully matches the applicant's official records.
